We at Fifth Street would like to congratulate you on this special occasion in your lives. Whether planning a small intimate or grand formal ceremony, we want this day to be glorified by God. Our wedding coordinator, Ms. Ora Kenney is responsible for assisting you in planning your special day.

Please complete this form and return to the church office with a $50.00 non-refundable deposit (to be deducted from your total fees). This will be used for setting up your initial meeting with Ms. Kenney and to also secure your date/time on the church calendar.
